Open Enrollment runs November 1, 2011 through December 16, 2011 for plan changes effective January 1, 2012.

During Open Enrollment, each member receives information in the mail regarding the opportunity to change your plan selection. In the event you are not eligible for Trust benefits during an annual open enrollment period, you may change your plan selections as of the date you re-establish eligibility, provided you were eligible for the eligibility quarter preceding and the eligibility quarter after the Open Enrollment period. Plan changes cannot be made at any other time unless you move out of your current HMO's service area.

Please Note: ALL DENTAL AND VISION PLANS WILL BE ELIMINATED EFFECTIVE JANUARY 1, 2012 IN ALL AREAS.  (Only vision exams will continue to be available under the medical plans except the Fee-For-Service plan).

All new or reinstated eligible's must enroll in an HMO plan unless they reside outside an HMO service area until the next scheduled open enrollment period.
