Hours worked in September through February are paid on July 1st. To receive a vacation check, you must complete and return the Vacation Request form by June 15th.

Hours worked in March through August are paid on December 1st. To receive a vacation check, you must complete and return the Vacation Request form by November 15th .

Be sure that you save all your check stubs and that your stubs have the employers name, your name, and the work period. Most employers will report hours to the Administrative Office according to payroll periods and not according to actual days worked, so that hours worked in the last week of the month may be reported in the following month. The hours worked during the last week of August 2006, for example, may be paid to you in the July 2007 payout.

Have you claimed all of your vacation funds? If you do not file a "Request for Benefit" form within two years after the original July 1 or December 1 distribution date applicable to the amount of vacation benefit, you are deemed to have elected to contribute the amount of such benefit to the Plan, and release to the Plan all right, title and interest in and to such payment.

If you have reason to believe that your benefit check has been forged and cashed by another person you will need to file an Affidavit of Forgery. This form will need to be completed and signed in the presence of a Notary Public. Click here to print form.
