		DentalSource Effective May 1, 2009, Southwest Carpenters is offering Preferred Elite EPO Dental Plan made available from DentalSource Dental Plan, Inc. (administered by Total Dental Administrators). The Preferred Elite EPO Dental Plan is an Exclusive Provider Organization (EPO) that provides covered services from an exclusive list of contracted providers. You may visit any contracted provider from the participating provider directory for covered services and you can decide whom to see at the time of service. Just call and make your appointment, visit the contracted provider, and pay the required copayment for covered services. There is no deductible to meet and the plan provides a $1,500 annual benefit maximum per covered person. Orthodontia coverage for eligible dependent children is also provided at 50% coinsurance to a $1,000 lifetime maximum benefit. Click here for the Plan Summary. Click here for an Enrollment Form. 		DeltaCare USA Effective January 1, 2008, DeltaCare® USA --Plan 11 A -- (administered by Delta Dental of California) provides you and your family with quality dental benefits at an affordable cost. The DeltaCare USA program is designed to encourage you and your family to visit the dentist regularly to maintain your dental health. When you enroll, you select a contract dentist to provide services. The DeltaCare USA network consists of private practice dental facilities that have been carefully screened for quality.
