You have 2 medical plans to choose from.  You may make this choice when you become eligible for benefits or during Open Enrollment.   All new or reinstated eligibles must enroll in an HMO plan unless they reside outside an HMO service area until the next scheduled open enrollment period.

Blue Cross and Blue Shield of New Mexico is a Health Maintenance Organization (HMO) option.   You must go to their medical clinics and hospitals.  To contact Customer Service, call (800) 423-1630. To go to the web site or to locate a provider, click here.

Click here for a Blue Cross and Blue Shield of New Mexico Enrollment/Change form. IMPORTANT!  THIS FORM MUST BE SUBMITTED TO CARPENTERS ADMINISTRATIVE OFFICE, NOT TO Blue Cross/Blue Shield.

 

The Fee-for-Service Plan allows you to go to any physician or any hospital.  If you use a physician or a hospital that is on a preferred provider list, your out-of-pocket costs are reduced.  To contact Customer Service, call 800.303.5921Blue Shield of California administers the Fee-for-Service Plan health plan on behalf of SW Carpenters; however the network used will remain the same Blue Cross Blue Shield of New Mexico providers you are currently using.
