The Fee-for-Service Plan allows you to go to any physician or any hospital. However, if you use a physician or a hospital that is on a preferred provider list, your out-of-pocket costs are reduced.
Effective June 1, 2010, the Trust has contracted with Anthem Blue Cross to provide a national PPO network of providers called BlueCard. This change will provide you with greater access to PPO providers near your home, work, and across the country.
The BlueCard network consists of hospitals, physicians, home health care providers, laboratories, durable medical equipment providers, physical therapists and other allied health care professionals under contract with Blue Cross organizations throughout the country.
